Understanding failure is the key to advancing patient safety. In the FaiLure Analysis Workshop, we create a confidential, protected peer-review environment where you can openly discuss your most challenging cases—your "nightmare scenarios"—and uncover critical insights to improve outcomes.

Confidentiality Matters: To ensure a safe space for open discussion, all participants will sign a non-disclosure agreement (NDA).

*FLAW case eligibility:

  • Submissions must describe a real complication with a severe, poor outcome, such as death, stroke, or permanent organ damage.
  • Minor complications will not be accepted.
  • Surgery alone is not considered a failure, even when performed emergently.
  • “Great saves” are not appropriate; FLAW is not intended to showcase successful recoveries.
  • The case must be your own case of failure.
  • Any catheter-based intervention is eligible, including structural, congenital, valvular, coronary, or peripheral procedures.
  • Case selection is competitive, as submissions typically exceed available presentation slots. Final decisions are made during the session.
  • Multiple cases may be submitted, but each presentation must focus on one case only.
